چکیده

The development of bronchial and vascular reconstruction techniques enables us to carry out lung resections that save the parenchyma. In 1947, Price-Thomas1 performed the first sleeve lobectomy in a bronchial adenoma. Later, Allison2 in 1959 reported the first case in a patient with carcinoma while also performing reconstruction of the pulmonary artery that had been infiltrated by the tumor. Our patient is a 78-year-old male with a smoking history of 80 pack-years who had developed COPD and was being treated with bronchodilators. On the chest CT, we observed a 4-cm tumor in the LUL that was in close contact with the bronchus of the lower lobe. Bronchoscopy demonstrated that the tumor was obstructing the upper left bronchus and extended toward the bronchus of the LLL. On PET, the lesion presented an increased metabolism with bilateral perihilar and hypermetabolic mediastinal lymphadenopathies. Ultrasound-guided bronchoscopy ruled out metastasis in said lymph node stations.

Bronchovascular reconstruction with a bovine pericardial conduit and surgical reintervention due to thrombosis with revascularisation.

We present the case of a 57-year-old male with left hilar squamous cell carcinoma infiltrating the pulmonary artery and in whom a sleeve bronchoplasty and angioplasty were performed using a bovine pericardial conduit. Three days post-operatively, graft thrombosis was detected; thrombectomy and graft reconstruction were performed with revascularisation of the graft.

A New Brace for Maintaining the Neck in a Suitable Position Following Tracheal Reconstruction

Segmental resection and end-to-end anastomosis is the treatment of choice for patients suffering from tracheal stenosis for whom conservative management is not planned. A complication of this procedure is tension-induced anastomotic failure. To prevent this complication, maintaining the neck in full flexion by means of a suture between the chin and upper chest is a traditional approach. We have...

An analysis of variations in the bronchovascular pattern of the right upper lobe using three-dimensional CT angiography and bronchography

OBJECTIVES General thoracic surgeons must be familiar with anatomical variations in the pulmonary bronchi and vessels. We analyzed variations in the bronchovascular pattern of the right upper lung lobe using three-dimensional CT angiography and bronchography and then compared our results with those of previous reports. METHODS We reviewed anatomical variations in the right upper pulmonary bro...
